CORE-FDM is offering a discount for offices with multiple registrants from the same office location. Here is the breakdown:

 

  • 3 to 4 registrants from same office - $100 off each registration (member or nonmember rate)

  • 5 and over - $150 off each registration (member or nonmember rate)

 

Discount will be applied after registrations have been verified and depending on the registration type, the appropriate refund will be issued through PayPal back to the registrants from same office location. Cancellation Policy 

Full Refund on or before 10/19/2018, 50% Refund 10/20/2018 - 10/26/2018.  No refund after 10/27/2018. However, course fee can be applied to future course. If course is cancelled by

CORE-FDM at any point, a full refund of course registration will be offered.

 

CME Hours
The American Fascial Distortion Model Association (AFDMA) has requested that the AOA Council on Continuing Medical Education approve this program for 20 credits of AOA Category 1A CME credits.  Approval is currently pending.

 

All other healthcare professionals must submit their seminar certificate to their licensing organization to receive CEUs.

 

Physicians should claim only the credit commensurate with the extent of their participation in the activity.

 

Hotel Reservations

CORE-FDM has not established a room block at any hotel in Honolulu allowing registrants the opportunity to stay anywhere.  It is highly recommended that a hotel reservation be made as soon as you register as hotel rooms are sometimes very hard to come by in Honolulu and surrounding area at that time of the year.
