CORE-FDM is offering a discount for offices with multiple registrants from the same office location. Here is the breakdown:

3 to 4 registrants from same office - $100 of each registration (member or non-member rate)

5 and over - $150 off each registration (member or non-member rate)
Discount will be applied after registrations have been verified and depending on the registration type, the appropriate refund will be issued through PayPal back to the registrants from same office location. Cancellation Policy
Full Refund on or before 1/10/2019, 50% Refund 1/11/2019 - 1/25/2019.
No refund after 1/25/2019. However, course fee can be applied to future course.
Hotel Reservations
CORE-FDM has not established a room block at any hotel in Anchorage allowing registrants the opportunity to stay anywhere. Suggested place for lodging is Dimond Hotel, 700 E Dimond Boulevard Anchorage, AK 99515
CME Hours
The AFDMA has requested that the AOA Council on Continuing Medical Education approve this program for 20 credits of AOA Category 1A CME credits. Approval is currently pending.
All other healthcare professionals must submit their seminar certificate to their licensing organization to receive CEUs. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
